I need to cancel or reschedule an appointment

  • Please contact the clinic as soon as possible to either make changes or cancel your appointment.

  • In order to ensure we are able to offer timely and urgent appointments to waitlisted patients, ALL cancellations with less than 48 business hours notice will incur a fee. Please refer to our Cancellation Policy for more details.

I need a Telehealth appointment

When you receive a reminder for your appointment, please ensure that you specify if you would like this to occur via Telehealth. You can also email us on reception@etonplace.com.au to let us know. Please be advised that Telehealth consults must be prepaid.

I need my therapist to complete paperwork 

  • Please complete this form.

  • We will forward this to the relevant practitioner.

  • Please note that you may be required to schedule an appointment to complete your paperwork.

  • All letter/report/ paperwork requests outside of scheduled appointments may incur a fee.
