I need to cancel or reschedule an appointment

  • Please contact the clinic as soon as possible to either make changes or cancel your appointment.

  • In order to ensure we are able to offer timely and urgent appointments to waitlisted patients, ALL cancellations with less than 48 business hours notice will incur a fee. Please refer to our Cancellation Policy for more details.

I need a script

  • Please complete this form.

  • Please note that, depending on circumstances, it may not be appropriate to issue a script without an appointment. 

  • All script requests outside of scheduled appointments may incur a fee.

I need a Telehealth appointment

When you receive a reminder for your appointment, please ensure that you specify if you would like this to occur via Telehealth. You can also email us on reception@etonplace.com.au to let us know. Please be advised that Telehealth consults must be prepaid.

I need my doctor to complete paperwork

  • Please complete this form to organise this.

  • Please note that you may be required to schedule an appointment to complete your paperwork.

  • All letter/report or paperwork requests outside of scheduled appointments may incur a fee.

  • To ensure sufficient time allocation, please ensure you let reception know before your scheduled appointment if you will require paperwork to be completed.

I need an urgent appointment

  • Please contact reception as soon as possible to let us know if you need to see your doctor urgently. We will put you on an urgent waitlist and try to offer you an appointment as soon as possible. Please note that if you require an urgent appointment, you may need to work around the available times we can offer you, as these will be limited.

I need to speak with my doctor outside of my appointment

  • Please contact reception to let us know this and we will let your doctor know. Please note that different doctors offer patients different options for communication outside of appointment times and these will be given to you when you book in for your initial appointments. Whilst we are very mindful of your privacy when forwarding your requests and correspondence to your doctor, the more information we can give your doctor, the better able they will be to support and assist you.
